論文発表

論文発表はその人が研究者としてどのようなものに関わってきたかを知るうえで有用と考えるため掲載します。一般的に言うと、論文は今までにない新しいことを発表するるためのツールですので、例えば「内視鏡を1万件やりました」「クローン病患者さんを1000人診察しました」などのようなことは論文にはなじみません。このような意味で、私がこれまでやってきたことの一端をお示しする目的でご紹介させていただきます。

この論文は私が初めて書いた英文論文です。執筆に１か月を費やしました。手術して直腸だけ残した潰瘍性大腸炎の患者さんの直腸に重症の潰瘍性大腸炎のような潰瘍ができましたが、それは潰瘍性大腸炎の炎症によるものではなくサイトメガロウイルスの再活性化による潰瘍であると分かり、サイトメガロウイルスの治療をしたら治癒した、という患者さんの報告です。したがって、潰瘍性大腸炎の重症例ではサイトメガロウイルスが悪さをしているものがあり、サイトメガロウイルスの治療をすれば手術しなくてもよい患者さんが含まれているかもしれないので注意しましょう、と呼びかけました。幸い、その雑誌の編集者が取り上げてくれ、以降、潰瘍性大腸炎がよくならないときにはサイトメガロウイルスのチェックをするようになりました。

で後輩の小島先生が手術症例などで検討し、やはりサイトメガロウイルスのチェックは重要ではないかとの結論です。

【掲載外】篠崎　大．クローン病と下部消化管癌―本邦の現況－　日本大腸肛門病学会雑誌61(7): 353-363, 2008.

これは、日本語の論文です。その頃、私は学会発表を聞いていて、クローン病に合併した大腸がんなどの悪性腫瘍の報告が増えているような印象がありました。それまで潰瘍性大腸炎では大腸がんの合併が多いと言われ対策のため内視鏡が勧められていましたが、クローン病ではあまり言われていませんでした。調べてみると、確かに目立って増加しています。とくに日本では直腸や肛門周辺のがんが圧倒的に多かったのです。欧米ではクローン病の大腸がん合併は議論されていましたが、どちらかというと大腸の奥の方（盲腸や上行結腸など）が多く日本のパターンとは大きく異なります。そこで、症例報告を１例１例集めて集計し、日本大腸肛門病学会雑誌に投稿しました。通常、新しい発見は英文で発表すると全世界的にアピールできますが、今回は日本でクローン病患者さんががんで亡くなる人を少なくしようとの意図で論文を書いています。そのため、敢えて日本語で書きました。それから数年は、学会でクローン病に合併した大腸がんについてのトピックが取り上げられ、重要性が認識されたので、恐らくたくさんの患者さんが早期に診断され、命が救われたのではないかと考えています。

私にとって、目の前の患者さんを治療することはもちろん幸せなことですが、私の論文を読んだ先生がご自分の患者さんの診療に活かしていただくことによって、私がお会いしたことのない患者さんにも間接的にお役に立てれば幸せだと思っています。
